As mentioned previously, you will need to have Suchdol in the Kuttenberg region unlocked to start Mysteria Ecclesia DLC in KCD2. To do so, you will have to reach The Sword and the Quill’s second day. Once you have done so, a purple icon marking the DLC should appear in Suchdol. You have to find Peter of Pisek and interact with him. This will start the Royal Physician quest and the DLC. Your first task is to find Albich of Uniczow after getting the Letter of Introduction from Lord Pisek.

Find Albich of Uniczow

Go to Kuttenberg and reach the blue marker (quest location) during the daytime. Inside, ask the trader behind the counter about where Albich of Uniczow is, who will tell you that Albich can be found two houses to the east. You can refer to your map for the location.

Once you reach the location, you will see Matthew Ram of Tseyle and his bodyguard there. Go near them and “Investigate”. The new map marker will take you to the house’s south entrance. Follow it to enter the house and go upstairs.

Talk to Albich

Wait for Albich to finish treating the maid (doing otherwise will result in you interrupting him and losing reputation). Follow these prompts:

• (Be quiet)
• (Still silent)
• Here’s Pisek’s letter.
• Aren’t you afraid to go alone?

Deal With the Guards and Go to the Monastery

Once the cutscene ends, you will have to deal with the guards. Depending on your Intimidation, Impression, or Scholarship stats, you will have multiple options to deal with them. If nothing works, you can engage in violence to dispatch them. After that, talk to Albich to begin your journey to Sedletz monastery. This is a point of no return. If you choose to go with Albich to the monastery, you will have to complete the DLC storyline before being allowed to return to the open world.

Talk to Father John

At the monastery, choose the following prompts with Father John to learn about the plague:

• Does it look like the plague? → I’m sorry.
• Who are the sick? → What’s bothering you, Albich? / If you have a high enough Scholarship stat, you can choose “How could they all have got infected?”
• Where could the plague come from here?
• What now, Albich? → Alright.

This will conclude the Royal Physician quest in Kingdom Come Deliverance 2 Mysteria Ecclesiae.
