In Baldur’s Gate 3, The Amulet of Elemental Torment is a jewelry item available from Brem, the vendor in Act 1. When the character wearing this item stands on certain surfaces like fire and deals damage to someone, the target they’re attacking also feels the effects of that surface. In this case, they would also take damage from the fire. It’s like the environmental damage is reflected back onto the target. It’s use is very situational and will work on specific builds.
The Amulet of Elemental Torment can complement sets like Wavemother’s Robe, The Sparkswall, and the Watersparkers. The Watersparkers electrify the water when the wearer stands in it, Wavemother’s Robe can heal you while they’re in the water, and the Sparkswall grants lightning damage resistance. When you wear all four sets together, they create a combination of area-of-effect elemental status effects. This combo can not only damage enemies in water but also passively heal you at the same time.
You can equip it to your character’s neck slot in the inventory, providing additional bonuses and buffs. There are no requirements for this item. You can equip it on any class, companion, or character.

Enter the entrance to the Zhentarim Hideout through the barn at the back of the Waukeen’s Rest mansion, near Waukeen’s Rest fast travel portal. However, before Brem opens his shop to you, you must complete the Missing Shipment Quest first.
Emerald Grove
It’s also possible that Arron in Druid Grove will have this item. Amulet of Elemental Torment you can buy from Arron in Emerald Grove during Act 1 from the vendor, named Arron. He stands just next to the entrance to the caves that give shelter to the Tieflings and settlers. All you must do is find the Druid Grove and help defend it from Goblins, and you will be allowed to enter the location straight after the prologue, where you can find the item.
