In Borderlands 4, you can obtain the Teen Witch Class Mod by farming the Primordial Guardian Radix boss inside the Arch of Radix Vault, located in Grindstone of the Worthy within the Carcadia Burn region.
However, the Vault is only accessible once you collect three Vault Key Fragments, scattered across the area. After the location unlocks, you can head to the location to encounter the required boss. Defeating Radix is definitely one of your toughest challenges in the game, as you need to clear a total of three arenas containing powerful enemies before you encounter Radix. To tackle the boss easily, you need to use weapons with Shock and Corrosive elements.
