Farm From Urivar or Echo of Varshan
The greatest chance of acquiring the item comes from defeating world bosses. Urivar and Echo of Varshan are your best bet for getting Balazan’s Maxtlatl. However, there are some conditions that you’ll need to follow. Firstly, you have to face these bosses in at least the Torment 1 difficulty setting to get the uniques from them. Higher difficulty settings will increase the chances of the unique item dropping. Also, you’ll get more drops from each encounter with these bosses.
You will need to spend 12x Malignant Heart to fight Echo of Varshan each time. For the Urivar boss fight, you’ll spend 12x Juticator’s Mask. So, make sure you have plenty of these items in stock before you begin farming the unique pants.

The perk of the unique is that it damages Close Enemies for 200-300% of your Thorns. This damage also applies as Poisoning for a span of 3 seconds. If you retaliate with Thorns, each time the Poisoning damage will increase by 100% (stacks up to 300%) for 5 seconds.
