You get the Liminal Echo by crafting it using 3000x Horadric Phials, 15000 Emerald Fragments, 15000 Sapphire Fragments, and 15000 Ruby Fragments at the Horadric Lapidary. Before you craft the item, you need to unlock its recipe first through the Horadric Tomes. The steps below should help with that:
- Interact with a Nightmare Sigil to enter a Nightmare Dungeon.
- Proceed through the Nightmare Dungeon and use the Horadric Wayfinder tool to find a Horadric Strongroom.
- Enter the strongroom and kill as many enemies as you can within the timer. Collect Attunement, and the meter to your right will fill up.
- Once the timer runs out, a boss will spawn. Beat this boss to complete the Horadric Strongroom. This will drop a loot cache with a Horadric Tome inside. This tome can contain the recipe for the Liminal Echo, which you can use to craft.
