Talk to Betrys at the Village

While exploring the Village at the far southwest in the Horns of the South, you will notice a woman named Betrys standing near a cage holding a man. Interact with Betrys to learn about the strange dream that the villagers had. She also reveals that Col, her beloved, bit one of the keepers and has been imprisoned. You will start the quest by choosing the dialogue option related to their camp, where the strange dreams originated.
Reach the Camp and Sleep in the Bed
Betrys informs that their camp is located near a mysterious structure called Archspire. Follow the quest marker to reach the camp, located near the King’s Road. You will encounter a Corpse Eater inside the camp. Defeat the Corpse Eater and then interact with the nearby bed to sleep. This teleports you to a strange dimension—the source of the mysterious disease.
Confront Abhartach

As you move through the strange dimension, you will hear the haunting voice of a Fore-Dweller called Abhartach pleading for help. Follow the marker to reach the Abhartach. Upon interacting, you’ll be presented with two choices:
- Let Abhartach bite you: It dies from your blood.
- Refuse the request: This triggers a battle with Abhartach. However, it doesn’t pose a threat and can be taken out easily.
The Abhartach dies either way, regardless of your choice, and the source of the disease is eliminated. Pick up the Fore-Dweller Bauble from its corpse and follow the marker to exit the dimension.
Return to Betrys
Upon exiting the strange realm, return to Betrys. She will tell you that the Keeper has recovered, though Col is losing his sanity. She acknowledges that you have destroyed the source and shows her gratitude, which completes the quest. If you have a high Spirituality, you can also convince Betrys to end Col’s suffering, which rewards additional gold and XP.
