As you explore the ancient Greek-inspired world in Titan Quest 2, you will not only encounter various mythological creatures but also unique quests. The World Quests are the game’s side quests that delve into the lives of the people around the land. Completing these quests can significantly aid your progression by rewarding XP, loot, and valuable gear.
The Headhunt is one of the World Quests available in the Titan Quest 2 early access. It revolves around the militia, where the commander asks for your help to stop the invasion of Ichthian forces in the Flooded Farmlands.
Your primary goal is to find and slay the four Ichthian Commanders in different directions around the Flooded Farmlands and collect their trophies. Here is a complete walkthrough on how to complete the Headhunt Quest in Titan Quest 2.

Headhunt Quest Walkthrough in Titan Quest 2
The Headhunt World quest begins after you talk to the Rizon, the militia commander, at the Pyrgos Village. After your conversation with Lochagos Rizon, head to the padlock on the left and defeat the Icthian Brute. Then, Rizon provides you with the coordinates of the four Ichthian Commanders. Simply killing the commanders is not enough, as you must also collect their medallions as proof.
South Ichthian Commander
You will find the first commander in the Foggy Hideout camp at the Southern Flooded Farmlands. There you will encounter Ichthian-Scales-Like-Shell, a level 9 enemy with poison-based attacks. Defeat the enemy and collect the Commander Medallion, which updates the quest to the next target.
West Ichthian Commander
Head your way to the Western Flooded Farmlands and get to the Butcher’s Fort camp. Here you will encounter Ichthian-Claws-of-Crimson. The level 11 enemy uses frost attacks to freeze your movements. Once you defeat the Ichthian commander, collect the Commander Medallion to proceed.
East Ichthian Commander
You can find the third Ichthian Commander at the Northern Flooded Farmlands within the Roadside Encampment. Fight and defeat Ichthian-Bile-that-Burns, a level 14 enemy. Similar to the first commander, this one also relies heavily on poison attacks. After defeating the commander, fetch the Commander Medallion to move on to the final one.
North Ichthian Commander
The fourth and final commander is located at the Northern Flooded Farmlands. As you pass through the Secluded Beach, you will reach the Clifftop Outpost. Proceed on the Clifftop Outpost, and you will face Ichthian-Fist-That-Breaks. This level 15 enemy boasts high raw strength, dealing heavy damage. As you defeat the commander, collect the final Commander Medallion trophy.
Once you collect all four medallions, return to Pyrgos Village via the waypoint and report to Lochagos Rizon. Interact with him to deliver the news and hand over the medallions as proof of death. The impressed militia commander will reward you for your effort, thus concluding the quest.
