| Chapter | Location |
| Chapter 1: Village of Khalim | During the “Destroy the demonic portals” objective, head to the northeastern cliff and scale up the rocks to find the Codex. During the “Defend the Beach” objective, you will find a scalable wall after the demon horde. Head up to find the Codex. |
| Chapter 2: Hebeth | After beating the Imps, enter the room with the green LED lights. Take a left and jump over the boxes to find the Codex. |
| Chapter 4: Sentinel Barracks | Take a left just before you enter the Dragon’s Den. |
| Chapter 5: The Holy City of Aratum | Under the metal railing near the third Hell Carrier. Take down the fourth Hell Carrier and enter the chamber with two portcullis gates. Head right, past the scaffolding, and push the box ahead. Scale up the box and the scaffolding beside it to reach the Codex. |
| Chapter 6: Siege – Part 1 | Take a right from the crossroads where you see the giant sword statue. Scale the wall to your left to find the Codex. Take a left from the junction where you see the Titan demon and climb the rock to find the Codex. |
| Chapter 7: Siege – Part 2 | Use the pink key on the gate in the courtyard to find the Codex. |
| Chapter 8: Abyssal Forest | Keep your eyes peeled to the right while heading through the barracks. You will find a breakable section of vines. Break through to obtain the Codex. |
| Chapter 9: Ancestral Forge | Head to the northwesternmost end of the map and drop down from the edge onto a platform below. You will find the Codex here. On your way to the Ancestral Forge, you will find a huge chamber with blue LED lights. Grapple above to find the Codex. |
| Chapter 10: Forsaken Plains | Past a pink gate on your way to the second Hell Cannon. On your way to the second Hell Cannon, take a right from the Sentinel Shrine. Follow along the path past the flamethrower traps to find the Codex. |
| Chapter 12: Sentinel Command Station | Keep your eyes peeled as you move towards Serrat. While passing through the red vents, you will find the Codex past a broken wall to your right. |
| Chapter 14: Spire of Nerathul | You will find the Codex on your way to the third transmitter. Take a right past the waterfall and follow this path. Hop off the dragon at the very end and exit the cave. Stick to the left outside and drop down to the side of the cliff to find the Codex. It’s next to the elevator that leads you to the Old One. |
| Chapter 15: City of Ry-uul | On your way to activate the portal, use the jump booster to jump up. Instead of going straight to the marker, drop down the ledge to your left and follow the path nearby to find the Codex. |
| Chapter 16: The Kar’Thul Marshes | As you make your way to the second Relic Fragment, keep your eyes peeled on the left. You’ll find a cave with a glowing blue switch. Hit the switch and turn back to find an unlocked door with the Codex. |
| Chapter 17: Temple of Lomarith | Inside a shipwreck in the underwater section of the chapter. |
| Chapter 18: Belly of the Beast | You will find the Codex in a small recess of the walls right after the “Destroy the Organ” objective. |
| Chapter 19: Harbor of Souls | Along the main path towards the Barge. Keep your eyes peeled to the left as you approach the gate with twin statues and blue braziers. When you reach the large chamber with the chairs on your way to the helm, look to your right for a path to the Codex. |
| Chapter 20: Resurrection | Look for a gold-filled path when you reach the arena with the crucifixes. You’ll find a yellow key here. Drop down the ledge and unlock the gate in front of you to find the Codex. It’s on the edge of Kreed’s ship, right before the entrance. |
| Chapter 22: Reckoning | The Codex is along the main path, early in the level. |
