Fugitive Heads are end-game crafting materials, that can only be farmed at higher difficulty levels, specifically Torment. They drop from Headrotten enemies whose names include “Fugitive Head,” such as Cunning Fugitive Head or Enraged Fugitive Head. These Fugitive Heads are higher-level bosses who appear at the Headhunt zones and Roothold Dungeons. White Rootholds are smaller dungeons and easier to complete than Headhunt zones, they have a low spawn rate of Fugitive Head Headrotten bosses.

Torment 1 has a 4% chance of Fugitive Heads spawning during Headhunts and inside Rootholds. This percentage increased by 2% for every subsequent Torment difficulty tier with Torment 4 caps at a 10% chance. Consuming the Draught of Whisper Elixir further increases the spawn rate of Fugitive Heads by 50% for the duration of one hour.

How To Use Fugitive Heads in Diablo 4?

As mentioned above, Fugitive Heads serves as an essential component for crafting Magic Occult Gems. Alongside Fugitive Heads, you need Restless Rot and Gem Fragments to craft a Magic Occult Gem.

Once you have the required materials and learned the recipe, bring them to Gelena at the Tree of Whispers to craft Occult Gems. You can also use Fugitive Heads to trade for Restless Rot at the Tree of Whispers. However, it is not an effective way to use these rare resources.
