In Baldur’s Gate 3, Horns of the Berserker is a cloth helmet and you can buy it from Entharl Danthelon vendor in Wyrm’s Crossing during Act 3. It offers increased damage for enemies who’ve already taken damage. The Seeking Blood special feature gives the wearer a +2 bonus to attack rolls once an enemy has taken damage. The Blood Thirst feature gives additional necrotic damage if you don’t have full health. If you have full health, Horns of the Berserker will cause you to take 1d4 necrotic at the end of your turn.
The Horns of the Berserker synergies well with aggressive melee damage builds. In particular, the Barbarian and Monk make the best use of this item. Barbarians are required to attack or be hit every turn to keep their Rage active. While Monks look to add unarmed damage from cloth, and extra necrotic damage synergies well with any build.
You can equip it to your character’s chest slot in the inventory, providing additional bonuses and buffs. There are no requirements for this item.

How to Get Horns of the Berserker in Baldur’s Gate 3
You can buy the Horns of the Berserker from Entharl Danthelon in Danthelon’s Dancing Axe, during Act 3 in Baldur’s Gate 3. At the beginning of Act 3, in Rivington, head north. You’ll encounter a city street on a bridge leading to Wyrm’s Rock Fortress. Danthelon’s Dancing Axe house is in the last house on the northwest side of Wyrm’s Crossing. Engage with Entharl Danthelon and purchase the Horns of the Berserker from him for 299 gold.
If you’ve recruited Jaheira, you’ll also be directed to the same house basement to gather information from the Harpers about the Baldur’s Gate and Gortash situation.
It’s a vendor rotation item, so the vendor may not have it available immediately. You may need to take a long rest and return to Danthelon’s Dancing Axe to check his new stock. He should have additional or different items each time, so it may take a few attempts before you can get it.
