The Duchess is in danger. Use this guide to solve the mystery in the Lady of Paranoia quest in Oblivion Remastered.
In Oblivion Remastered, the Lady of Paranoia is a side quest that is a part of the Shivering Isles DLC. The quest is connected to the Understanding Madness quest. In the Lady of Paranoia mission, Syl, the Duchess of Dementia, needs your help to uncover any treachery. It is not an ordinary quest, as you will require support from an NPC named Herdir.

How To Start the Lady of Paranoia Quest in Oblivion Remastered
To begin the Lady of Paranoia quest, go to the House of Dementia. Once there, locate and interact with Syl, the Duchess of Dementia. When you speak to her, she will ask you to investigate a possible conspiracy. She will also inform you about Herdir, who can accompany you as an assistant.
Investigating the Conspiracy
As mentioned earlier, meet Herdir at the torture chamber. After a brief conversation, he will begin following you wherever you go. To start the investigation, begin questioning the members of the court. First, speak with Kithlan in the throne room.
During this task, you can ask Herdir for help whenever a conversation reaches a dead end. Use the dialogue option, “I don’t care, Herdir, torture her.” Herdir will then use a powerful spell to force the target to confess.
Kithlan will advise you to speak with Anya Herrick. Locate her and use Herdir’s spell on her. She will then reveal information about Ma’zaddha. With this new lead, fast-travel to Crucible and confront some people to gather intel about the conspiracy. However, avoid talking to the guards. Soon, you will receive intel that Ma’zaddha will be at the sewer gate near midnight.
Arrive at the specified location and secretly listen to the conversation during the meeting. Once it ends, confront Ma’zaddha and Nelrene. Use Herdir’s spell to pressure Nelrene into revealing the name of the conspirator: Muurine.
Completing the Lady of Paranoia Quest in Oblivion Remastered
Next, go to Ma’zaddha’s house, where you will find him dead. Collect a key from his corpse, then head upstairs to Ma’zaddha’s room. Open the cupboard to find Ma’zaddha’s Crinkled Note and Nelrene’s Ceremonial Shortsword.
Now, locate Muurine and show her the evidence. She will then admit her involvement in the conspiracy. To complete the quest, return to the House of Dementia and report the entire investigation. Syl will then head to the basement to execute Muurine, concluding the quest.
