At the end of Chapter 17, he is consumed by the Old One while exploring one of the regions. Chapter 18 Belly of the Beast starts with the Slayer ingested by the Old One. The chapter focuses on the Slayer as he finds a way out of the beast and saves Thira.

Destroying the Major Organ and Finding a Way Out
The chapter starts with the Slayer being ingested by the Old One. Continue along the fleshy pathway to locate the creature’s major organ, protected by three tentacles. To make the major organ exposed and vulnerable, you need to find and destroy three connecting organs linked to each tentacle. Follow the objective marker to locate the first connecting organ, where you will encounter a horde and a Demonic Leader. Eliminating the Leader rewards with the Demonic Essence — Armor, permanently raising the maximum armor.
After destroying the connecting organ, a brief cutscene will show that one of the tentacles is retracting. Now, locate and destroy the other two by following the objective marker. Once you destroy all connecting organs, head to the major organ, which triggers a Point of No Return checkpoint. The Slayer uses the B.F.C.(Ballistic Force Crossbow) to tear through the major organ. Then, use the Environmental Shield Charge to get out of the beast.
Heading to the Temple To Save Thira
Once you get out, your next goal is to rescue Thira by heading to the temple. Make your way to the temple by fighting through waves of enemies. Upon reaching the temple entrance, another Point of No Return will trigger. After you reach the top of the temple, clear the demons. Thira has been immobilized and imprisoned with high energy. Approach her to complete the End of the Chapter objective, which triggers the cutscene.
Final Cutscene
When the Slayer tries to save Thira, the massive energy surrounding her attacks him, making him immobile and weakened. The Slayer also learn that Bishop Kreed has now sided with Prince Ahzrak and serves him. The Witch extracts the power source inside Thira and channels it into the Heart of Argent. Prince Ahzrak absorbs the immense energy and becomes empowered. The chapter contains a total of 153 Gold, 1 Ruby, 1 Demonic Essence, 6 Secrets, 1 Codex, and 1 Toy.
