If you are curious about the list of confirmed surprise appearances or cameos in Death Stranding 2 so far, here’s the list.

Continuing the legacy of the original game, Death Stranding 2: On the Beach will also have surprise appearances of various iconic media personalities. Some of the special guests that may have a crucial role in the story have been revealed in the recent trailer. However, we might be seeing more in the form of brief cameos soon.
Hideo Kojima, the genius behind the Death Stranding franchise, is a big fan of celebrities in various forms of media. The best part is that he is not afraid to show it. His games have a ton of cameos and easter eggs, making them quite fun to replay and discuss just to uncover these secrets. With that said, let’s take a look at some of the confirmed surprise appearances in Death Stranding 2.
Confirmed Surprise Appearances So Far in Death Stranding 2
We don’t have much to work on this one because there are only a few surprise appearances revealed for Death Stranding 2 so far. Still, let’s list down some of the ones that have already been confirmed:

With so many personalities wanting to make a brief guest appearance in Death Stranding 2, Kojima Productions had to reject a ton of these requests. But of course, Hideo Kojima himself made sure his favourite Hololive Vtuber is part of his next big game.
For context, a Vtuber is an online entertainer who does various content while using a virtual avatar. It’s often 2D animated, but there are some that are static or using motion capture technology to move their 3D avatars. Pekora is one of these entertainers, often playing video games or singing on her livestream.
On May 29, 2025, Kojima Productions posted a short clip of Usada Pekora thanking Sam, the protagonist, for his hard work in Japanese. In exchange, Sam waves, saying “Arigatou,” which means “thank you” in English. It’s a very brief clip, but Pekora might be one of the preppers in the game.
Ma Dong-seok
Ma Dong-seok is widely popular for his appearance in the iconic Korean zombie film Train to Busan. It looks like he will also be making a surprise appearance in the upcoming Death Stranding 2 game. Not only that, there are also hints of him appearing in another game Kojima Productions is making. Could it be the upcoming horror game OD or an entirely new unannounced title? We’ll just have to wait and find out.

This could be another cameo from personalities in the horror film scene. I’m starting to think Hideo Kojima is a big fan of horror, with Junji Ito appearing on the original Death Stranding. But for the sequel, we have Danny and Michael Philippou, the twins behind the supernatural thriller Talk to Me.
The two brothers began their journey as YouTube creators through their channel, RackaRacka. They became known for wild, over-the-top skits that blend intense stunts with graphic special effects. These videos are usually delivered with a comedic twist.
Their surprise appearance in Death Stranding 2 was revealed in Hideo Kojima’s post on X showing their in-game models, motion-captured. According to Kojima, the two characters will also have voiceovers, hinting that they might be able to interact with the player.
Conan O’Brien (Hinted but Not Confirmed)
Conan O’Brien made a cameo in the original Death Stranding, where he can’t seem to stop making jokes relating to otters. If you complete his quest, you receive a nice new hood to wear. Can you guess what it is? It’s an Otter Hood that allows you to move smoothly while in rivers.
In one episode of Clueless Gamer, Conan O’Brien jokingly told Hideo Kojima that he wanted to appear in the next game as a hero, not just some goofy NPC. Kojima responds by inviting Conan to visit his studio again, where they will use a face scanner one more time for their next game.
It wasn’t specifically revealed if it was Death Stranding 2. On top of that, his studio is working on multiple games at the time of the video’s upload. However, considering how Conan already made an appearance in the original game, there is a possibility that he will be making a comeback for the sequel. And possibly, more importantly, will Conan still give Sam another Otter Hood, or will it be another gag this time?
