All Mythic Unique Items and How To Get Them

Diablo 4 initially launched with eight mythic Unique items in the base game. Later, four came as part of Season 6 and the Vessel of Hatred Expansion, bringing the total count to 12. Hence, there are 12 Mythic Unique items in the game, all of which grant different status effects, incentivizing you to get them all. The Mythic Unique items in the game are as follows:

4Melted Heart of SeligGrants 60 maximum Resource. When taking damage, 75 percent is converted into 2 Resource for every 1 percent of Maximum Life lost.
5Ring of Starless SkiesSpending resources reduces resource costs and increases damage by 10 percent for 3 seconds, stacking up to 50 percent.
6The GrandfatherIncreases Critical Strike Damage by 100 percent. Other properties on this weapon can roll higher than normal.
7Ahavarion Spear of LycanderGrants a random Shrine effect for 20 seconds after killing an Elite enemy. This effect can occur once every 30 seconds.
8Tyrael’s MightWith full HP, your Skills unleash a divine barrage that deals damage.
9Heir of PerditionSuccumb to Hatred to earn Mother’s Favor, increasing damage dealt by 60 percent. Briefly steal Mother’s Favor from nearby allies by slaying enemies.
10Shroud of False DeathIf you haven’t dealt damage in the last 2 seconds, gain Stealth and 40 percent Movement Speed.
11Shattered VowEnemies afflicted by more Damage over Time than their remaining Life are Executed.
12Nesekem, The HeraldA random enemy is marked every 2 seconds, leaving it Vulnerable to Critical Strike and Overpower. You can hit a marked enemy up to 10 times. Hitting a marked enemy 10 times.

You unlock these items as drops after beating enemies and ladder bosses in the game’s Torment difficulty. Moving up from Torment 1 to Torment 4 increases your odds of getting the item, so we recommend doing so. Alternatively, you can craft this item at the Blacksmith using Resplendent Sparks.
