You can unlock the Coalesced Blood ability by simply investing points in the skill tree as a Necromancer. At the earliest, you will need to spend 16 skill points to unlock the ability. With two more skill points, you can upgrade this ability to its most powerful potential. At this stage, all your attacks will deal 18% more damage if you are in a healthy state.
How To Use Coalesced Blood Ability

Necromancers usually have an army of undead guarding them at all times, which can make it easier for them to maintain the Healthy state. Also, with the Blood Mist skill, you can become immune to damage for short spans. During these times, you can easily maintain the Healthy state and keep inflicting increased damage.
