In Baldur’s Gate 3 the Orphic Ring is an accessory that you can only get in the very late game of Act 3, after making a critical story choice. It grants you access to the Confusion spell, which you can cast as a 5th level spell once per Long Rest. If it lands successfully, enemies remain confused for three turns. It is a great area of effect crowd control spell and can be useful in the big final fights of the game.
The Orphic Ring is highly recommended for Life and Trickery Domain Clerics, as well as College of Valour Bards.

How to Get Orphic Ring in Baldur’s Gate 3
The only way to get the Orphic Ring in Baldur’s Gate 3 is to take it from the body of Prince Orpheus. You will not be able to acquire it until very late in Act 3, after you have decided to find and confront the Elder Brain. When you’re ready to approach the conclusion of the game, you can find the Elder Brain by taking a branching path north from the Temple of Bhaal waypoint.
At the very end of this path you will find a dock with a small Skiff which you can interact with to take you further in. You will arrive at a place called the Morphic Pool and while the path here will branch out somewhat, it is mostly straightforward leading you on. There will be a few fights in here, but nothing too challenging. The Elder Brain will speak from time to time and afflict your party members as they approach.
When you finally reach the brain, there is a small series of cut-scenes and dialogue. You’ll be making some attempts to use the Netherstones to subdue it. There are some Difficulty Checks along the way, though it appears that most of these you are supposed to fail so don’t stress about them too much. At a certain point the Emperor will pull you and your party back into the Astral Prism.
Choice In the Astral Prism
Jump down near to where Prince Orpheus is being held captive and interact with the Emperor again. He will tell you the Elder Brain has evolved and you will have to change tactics. This is a critical juncture in the game – you will have to choose between siding with the Emperor or siding with Orpheus.
If you choose to side with Orpheus (and you must have the Orphic Hammer to free him) then you CANNOT gain the Orphic Ring. If you ally with him and then try to pickpocket him later, he will not have it in his possession (I tried).
The ONLY way to acquire the Orphic Ring is to side with the Emperor and then loot the Ring from the body of Prince Orpheus.
