The Sentient Amulet is a unique Monk-centric accessory you can upgrade via quest, and you can find at the Adamantine Forge during Act 1 of Baldur’s Gate 3 (BG3).
Sentient Amulet
In Baldur’s Gate 3 the Sentient Amulet is a neck accessory with perks which are geared towards the Monk class. First, wearing this amulet will grant you access to the Shatter spell at Level 2, which you can use once per Short Rest. The Shatter spell causes 3~24 Thunder damage. Additionally, creatures made of inorganic material such as stone have Disadvantage on their Saving Throw. Next, the other helpful bonus is the ability to use the class action Ki Restoration (Lesser) once per Long Rest. This will return Ki Points to a Monk, which the class uses to fuel various abilities.
When you first find the Sentient Amulet it will be under a kind of ‘curse’. It is inhabited by a spirit which will inflict the following condition on the wearer.
High Spirits: The amulet’s spirit lingers, making everything seem a little more vibrant. Must succeed a Wisdom Saving throw when using the amulet’s power, or become Hysterical. Hysterical is an effect similar to that induced by Tasha’s Hideous Laughter, and lasts for one turn.
You can rid the Sentient Amulet of this condition by completing the Help the Cursed Monk quest. Not only will the High Spirits condition disappear, but Shatter can be cast as a Level 3 spell. Moreover, the class action Ki Restoration upgrades to the ‘Greater’ version.

How to Get Sentient Amulet in Baldur’s Gate 3
To get the Sentient Amulet in Baldur’s Gate 3 you will need to find the Adamantine Forge, defeat the Grym and then head west across the lava to find a chest with the necklace locked inside. From the Ancient Forge waypoint, head southeast down the large broken staircase to find the Forge and fight Grym. After the fight you should be able to go west, jumping from rock to rock to stay out of the lava. Be careful to stay out of the way of the lava elemental which is circling the large platform at the end.
This platform is your goal and once you’re safely there, you’ll find a skeleton next to an Adamantine Chest. You’ll need to pass a 20DC Sleight of Hand check in order to successfully open it. Inside you will find the Sentient Amulet.
Once you’ve found the amulet, your party will feel the urge to laugh. You want to try to avoid laughing and will need to pass a 10DC Wisdom check. If you can avoid laughing the spirit of the Amulet will appear. He will request your help getting rid of his curse. You can choose to ignore the quest and use the amulet ‘as-is’, but the High Spirits condition will likely prove troublesome. However, you cannot complete the Help the Cursed Monk quest until you reach Rivington in Act 3.

How to complete the Help the Cursed Monk Quest
In order to remove the curse from the Sentient Amulet and upgrade the item to Very Rare, you have to complete the Help the Cursed Monk quest in Act 3. Like the nice guy he is, the spirit wants to pass on his curse to his granddaughter so he doesn’t have to deal with it anymore. You can find Shirra at the Open Hand Temple in Rivington. When you get there speak to Sister Yannis, who will tell you that Shirra died the previous year. She was buried in the crypt underneath the temple.
Head back into the kitchen area to the north and there you will find an Ornate Wooden Hatch. This will take you down into the crypt. From here, enter the double doors to the south, then enter the door to the west to find the sarcophagus of Shirra. Be sure to interact with the plaque on the sarcophagus in the southwestern area of the room before you open the lid. Afterwards, you’ll again enter into a conversation with the spirit.
How to Remove the Curse on the Sentient Amulet
With his granddaughter dead, the monk’s spirit asks if the party might be willing to bear his curse instead. REFUSE. The only way to get the upgraded and curse free version of the Sentient Amulet is to refuse the spirit’s offer. If you accept, you have to pass two 15 DC Wisdom checks. And if you fail either one your character permanently looses 1 point of Wisdom! Moreover, the only rewards you’ll get for being successful is the Tasha’s Hideous Laughter action. It’s not worth the risk, so avoid this entirely.
The safer option, with the better reward, is to reject his offer. This will trigger a fight where he raises the three other corpses in the tomb to attack you. Defeat them, and the Sentient Amulet will loose its curse, and you will get the Level 3 Shatter spell and the Greater Ki Restoration!
