Where to Get Sickle of BOOOAL in Baldur’s Gate 3
In order to receive the Sickle of BOOOAL in Baldur’s Gate 3 you need to find your way into the Underdark during Act 1, then discover the location of the Festering Cove, and finally convince the god BOOOAL himself to share a bit of his power.
The easiest way to get to the section of the Underdark you need to be in is by using the Selunite Outpost waypoint. If you haven’t already unlocked this waypoint another way is by using the Well in the Blighted Village. Interact with it and you only have to pass a low DC roll to discover the bottom.

Travel down and you’ll be in the Whispering Depths. There are some spider enemies and a large, powerful Spider Matriarch boss down here. But on the west end of the caverns, you’ll discover an open chasm that is glowing green. Cast Feather Fall on yourself and your party, and then jump down in. Once you’re in the Underdark, head south. You can’t enter the Selunite Outpost from the front, so go around to the west and jump down on the ledges along the side.

The paths down here will eventually take you to the Arcane Tower. To find the Festering Cove, however, you’ll want to stop after you first climb down into the red glowing area with all the exploding mushrooms. Just over to the left you’ll see a little alcove. You can barely see it, but if you move your cursor over the area the Cragged Rock should highlight. There will be a couple of mushroom ledges along the side wall. Use a ranged weapon first to shoot the red/orange mushroom across the way. Then jump over and climb down the Cragged Rock.

Once you enter the Festering Cove you’ll find a gathering of fish folk, the Kuo-toa. Approach the red figure on the pedestal they’re facing and you’ll start a dialogue cut scene. Question the Kuo-toa about who they’re really worshipping and you can make them doubt their god. As their faith drops the figure is actually revealed to be a scheming redcap.
Agree with BOOAL’s suggestion that you are his ‘Chosen’ and the worshiping Kuo-toa will redouble their efforts. BOOAL will then reward you with the Sickle of BOOAL.
